D/Sir Please refer to newspaper dated 28.08.2011 (Tribune) and …...... (copies enclosed) wherein news regarding lack of infrastructural facilities in Trauma Centre Jalandhar had been published. I, being, citizen of India request to supply the following information under Right to Information Act, 2005 :-
1(a) Intimate date of setting/opening of Trauma Centre. (b) Amount spent (head-wise) in building, development of Trauma Centre. (c ) Amount spent (item-wise) on infrastructural items in Trauma Centre. (d) List of all the infrastructural and medical equipments available in Trauma Centre alongwith their model number and prices. (e) Provide list of all type of facilities and test available in trauma centre alongwith prescribed fees to be charged from the patients (f) List of items/equipments not available but required for trauma centre. Also intimate effort done to procure the required items/equipments (g) Intimate objective/purpose of creation of Trauma Centre. Also intimate situation when the patient is to be treated in Trauma centre. Supply attested copies all the instruction issued by the Govt/department on this issue. (h) Supply attested copies of circular/notifications/guidelines issued by Govt/deptt regarding functioning, working and treatment in Trauma centre. 2. Intimate name and designation of the officer who is incarge of Trauma Centre. Also intimate his qualification and training in trauma management. (b) Give a list of all the staff working in trauma unit alongwith their professional degrees? (c ) Intimate, if the staff deputed in Trauma units have sitting duties? 3. Supply list of all the major surgical procedures done in trauma unit from 1.4.11 to 30 .11.2011 alongwith amount charged in each case. (b) How many of these procedures were done within 12 hours of arrival of patient and how many later than that. (c) How many procedures were done after 72 hrs of arrival of patient?
5. Give a list of all elective non- trauma procedures done in trauma unit from 1.4.11 to 30.11.2011 and amount charged in each case.
6. How many serious patients were shifted to other wards within 24 hours of arrival? what were the reasons for shifting ? 7. List of cases where Patient was not required to be treated in Trauma but still treated there alongwith its reasons. 8. Intimate circumstances in which patient is required to be treated in Trauma Centre. 9. Supply details of tests which are referred or done from outside due to non availability in Trauma. (b) Supply working email address of all Medical Officers and Group-A/B Officers and Hospitals/dispensaries working under the direct/indirect control of Civil Surgeon, Jalandhar. 10.
